Healthy Families Case Manager/ Family Support Worker -
Contract
POSITION:
2015
RESPONSIBILITIES:
Provides
intensive home based case management services to children at risk of
serious emotional disturbance and their families. Promotes positive
mental health development, family functioning, and parent-child
interaction. Develops and implements individual service plans and
completes required documentation. Utilizes basic computer and word
processing skills. Assesses families to Determine appropriateness
for services. Links families with community services.
REQUIREMENTS:
Minimum Requirements
M1:
Bachelor’s Degree in Human Services (Psychology, Social Work,
Counseling, or other related field)
OR
Registered Nurse with a valid
license to practice in Virginia.
M2:
Must have at least one year experience providing direct service to
individuals with mental illness, intellectual disabilities, or
individuals receiving gerontology services or special education
services (which may include supervised internship, practicum, and
field experience) required.
M3:
Must have a valid driver’s license and safe driving record.
Preferred
Requirements
P1:
Experience in case management
P2:
Experience in service planning process
P3:
Home visiting experience
P4:.Experience
working with children five years old and younger.
Fingerprints,
State and FBI criminal record reports, drug test, and central
registry (CANIS) report will be required upon request.
HOURLY RATE:
$16.15
