Healthy Families Case
Manager/Family Support Worker
Position Number: 2096
Provides intensive home based case management
services to children at risk of serious emotional disturbance and their
families. Promotes positive mental health development, family functioning, and
parent-child interaction. Develops and implements individual service plans and
completes required documentation. Assesses families to determine appropriateness
for services. Links families with community services.
Bachelor’s Degree in Human Services (Psychology, Social Work,
Counseling, or other related field) OR
Registered Nurse with a valid license to practice in Virginia with one year experience providing direct service to individuals with mental illness, intellectual disabilities, or individuals receiving gerontology services or special education services (which may include supervised internship, practicum, and field experience)
Must have a valid driver’s license.
Work schedule first shift hours with some night and weekends

Experience in case management
Experience in service planning process
Home visiting experience
P5: Experience in working with children five and under
State and FBI criminal record reports, drug test, and central
registry (CANIS) report will be required upon request.
ANNUAL SALARY RANGE:
$34,252 - 53,948 Excellent fringe benefits.
