Community Outreach Worker
Under the supervision
of the Prevention Program Manager, identify uninsured individuals, who are
potentially-eligible for state-sponsored health insurance coverage programs
(Medicaid); work individually with identified individuals to assess eligibility
for state-sponsored health coverage programs; and, assist those individuals with
the application and enrollment process for those programs
Must have a valid driver’s license.
Three or more years’ experience in customer service preferred
Excellent organizational skills and attention to detail preferred
Established relationships with organizations that serve
potentially-eligible children and pregnant women preferred
State and FBI criminal record reports, drug test, and central
registry (CANIS) report will berequired upon request.
$31,669 - $49,877 Excellent fringe
Position is grant funded for a minimum of 2 years.
