Position Announcement

 

Child and Adolescent Case Manager

 

Position Number: 3344

 

Responsibilities:
Provides assessment, linkage, and advocacy services to individuals from birth to age eighteen with a documented diagnosis of a developmental disorder, developmental delay, or presence of a syndrome typically associated with a developmental disability. Acts as primary facilitator in the coordination and integration of services for assigned caseload.  Develops and implements Individualized Family Service Plans (IFSPs) and Individual Service Plans (ISPs). Completes required documentation and complies with all regulations applicable to program services.  Applies generally prescribed standard practices over a wide range of procedures with some latitude for decision making.

 


Requirements:
Minimum Requirements
M1:
Bachelor’s Degree in Early Childhood Education, Special Education, Human Services related field or any discipline or profession listed under Part C Personnel Standards
OR
Registered Nurse with a valid license to practice in Virginia.

M2:
Within two months of employment, must complete Early Intervention Training Modules, apply for, and obtain Early Intervention Case Manager certification or complete DBHDS Basics of Case Management Training Curriculum modules.
Preferred Requirements

P1:
Prefer that applicant meet Early Intervention Professional Practitioner Level for a discipline or profession as defined in Chapter 12 of the Infant and Toddler Connection of Virginia’s Practice Manual (Table A – Practitioner Qualifications and Responsibilities). Click HERE for more information.
P2:
Prefer at least one year experience working with children under the age of eighteen with developmental delays or disabilities, or experience working collaboratively with community agencies that provide services to children and youth.

Fingerprints, State and FBI criminal record reports, drug test, and central registry (CANIS) report will be required upon request.


 

ANNUAL SALARY RANGE:
$34,252 – $53,948.  Excellent fringe benefits.
