Position Announcement

 

Child and Adolescent Case Manager

 

Position Number: 3344

 

Responsibilities:
Provides assessment, linkage, and advocacy services to individuals from birth to age eighteen with a documented diagnosis of a developmental disorder, developmental delay, or presence of a syndrome typically associated with developmental disability.  Acts as primary facilitator in the coordination and integration of services for assigned caseload; develops and implements Individualized Family Service Plans and Individual Service Plans; completes required documentation; and complies with all regulations applicable to program services. 


Requirements:
Minimum Requirements
M1:
Bachelor’s Degree in Early Childhood Education, Special Education, a Human Services related field, or any discipline or profession listed under Part C Personnel Standards; or a Registered Nurse with a valid license to practice in Virginia
M2:
Within two months of employment, must complete Early Intervention Training Modules, apply for, and obtain Early Intervention Case Manager certification or complete Department of Behavioral Health and Developmental Services Basics of Case Management Training Curriculum modules.
M3:
Must have a valid driver’s license.

Preferred Requirements

P1:
Prefer that Early Intervention Professional Practitioner Level be met for a discipline or profession as defined in Chapter 12 of the Infant and Toddler Connection of Virginia’s Practice Manual. 
P2:
Prefer at least 1 year of experience in working with children under the age of eighteen with developmental delays.
OR

Prefer at least 1 year of experience in working with children under the age of eighteen with developmental disabilities.
OR

At least 1 year of experience working collaboratively with community agencies that provide services to children and youth.

Fingerprints, State and FBI criminal record reports, drug test, and central registry (CANIS) report will be required upon request.

 

ANNUAL SALARY RANGE:
$33,581 - $52,890.  Excellent fringe benefits.
