Case Manager - Part C
Position Number: 3376
assessment, linkage, and advocacy services to individuals from birth
to age three with a developmental delay, atypical development, or
diagnosed disabling condition recognized by the Part C Early
Intervention system. The Child and Adolescent Case Manager – Part C
will act as primary facilitator in the coordination and integration
of services for assigned caseload; develop and implement
Individualized Family Service Plans (IFSPs); complete required
documentation; and comply with all regulations applicable to program
services. Applies generally prescribed standard practices over a
wide range of procedures with some latitude for decision making.
M1: Bachelor’s Degree in Early Childhood Education, Special
Education, Human Services related field or any discipline or
profession listed under Part C Personnel Standards.
Registered Nurse with a valid license to practice in
two months of employment, must complete Early Intervention Training
Modules, apply for, and obtain Early Intervention Case Manager
P1: Prefer that applicant meet Early Intervention Professional
Practitioner Level for a discipline or profession as defined in
Chapter 12 of the Infant and Toddler Connection of Virginia’s
Practice Manual (Table A – Practitioner Qualifications and
Prefer at least one year experience working with children under the
age of eighteen with developmental delays or disabilities, or
experience working collaboratively with community agencies that
provide services to children and youth.

ANNUAL SALARY RANGE
$34,937 - $55,027 Excellent
