Child and Adolescent Case
Position Number: 3344
linkage, and advocacy services to individuals age two to eighteen with a
documented diagnosis of a developmental disability to include intellectual
disability, developmental delay, or presence of a syndrome typically associated
with a developmental disability. Acts as primary facilitator in the coordination
and integration of services for assigned caseload.
Develops and implement Individualized Service
Plans (ISPs); completes required documentation and complies with all regulations
applicable to program services.
generally prescribed standard practices over a wide range of procedures with
some latitude for decision making.
Bachelor’s Degree in Human
Bachelor’s Degree in related field
with at least 15 credit hours of coursework in human services
Registered Nurse with a valid
Registered Nurse license to practice in Virginia.
At least one year experience working with children under the age of
eighteen with developmental delays or disabilities, or experience
working collaboratively with community agencies that provide
services to children and youth.
State and FBI criminal record reports, drug test, and central
registry (CANIS) report will be required upon request.
