Child and Adolescent Case Management

Case management is a collaborative process which assesses, plans, implements, coordinates, monitors, refers and evaluates options and services to meet a child’s mental health or substance use needs. The child and adolescent case management program provides short-term case management to children ages birth through 17 years of age with a serious mental health or substance abuse problem diagnosed under the DSM-5.  Case managers support caregivers as they assist them in obtaining the resources necessary to meet the mental health or substance use needs of their children. The ultimate goal of case management is to help caregivers advocate autonomously for their child/children as they navigate the mental health system.
