Family Assessment and Planning Team (FAPT)

The Family Assessment and Planning Team (FAPT) is a multidisciplinary team that considers the needs and strengths of youths and families, identifies and reviews the service plan, and makes recommendations to ensure the plan builds on the family’s strengths and needs. FAPT consists of parent representatives, private providers, and staff from DPCS, Court Services, Social Services and Schools. The Community Policy and Management Team (CPMT) manages the local Comprehensive Services Act (CSA) program by coordinating agencies efforts, managing the available funds, and establishing local CSA program policies.


Eligibility

The program serves youth who are Danville City and Pittsylvania County residents under the age of 18, have serious emotional or behavioral challenges, need resources beyond normal agency services (non-Medicaid funded services), require special education through private day schools as determined through the youth’s Individualized Education Program (IEP), require foster care services or services to prevent foster care placement, are supervised by the Juvenile and Domestic Relations District Court, are wards of the Department of Juvenile Justice, or may be at risk of residential placement.  Families or local providing agencies can refer an individual for FAPT services.  However, the client must meet criteria to receive FAPT funded services.  FAPT/CPMT requires that other non-FAPT funded services have been attempted before utilizing FAPT funded services.


FAPT services provided by DPCS

Case Support-Community - manage all community based services recommended by the FAPT team for the child and family.

Case Support-Residential - manage the residential placement and process for the child and family. 

Intensive Care Coordination (ICC) - ICC in the High Fidelity Wraparound (HFW) Model provides a structured approach to care coordination that is designed for youth and families where the youth is in, or at risk of, an out-of-home placement. These are youth with complex, challenging behavioral health issues who typically represent the upper 10 – 20% of a “severity pyramid”.
