ADULT CASE MANAGEMENT
Case Management assists individuals with developmental disabilities and their families in accessing services, supports, and resources within their community. Case Management functions as the door through which an individual passes to access needed services.
Case Management is licensed by the Department of Behavioral Health and Developmental Services.
Case Management services include:
Determination of eligibility for Developmental Disability Waiver Services
Assistance in transitioning from state facility-based services to local community-based services
Identifying and reaching out to potential individuals
Assessing needs and planning services
Linking and assisting individuals in obtaining needed services, supports, and resources
Coordinating services with other providers
Enhancing community integration
Monitoring service delivery
Advocating for people in response to their changing needs
Medicaid Developmental Disability Waiver Services
Developmental Disability Case Managers determine eligibility for Waiver Services. In order to be eligible, an individual must have a diagnosis of a developmental disability and must meet functional and financial eligibility.
Discharge and Transition Services
Case Managers also assist individuals residing in Intermediate Care Facilities for individuals with intellectual disabilities and the state mental health facility that serves Danville and Pittsylvania County, Southern Virginia Mental Health Institute; in transitioning from state facility-based services to local community-based service
