Assertive Community Treatment (ACT)
Assertive Community Treatment (ACT) focuses on individualized community-based treatment, rehabilitation, and supports designed to meet the needs of adults with serious mental illness who often require 24-hour service availability to avoid hospitalization. Complete wrap-around services include intensive case management, psychiatric diagnosis and treatment, medication prescription, administration and monitoring, substance use services, counseling, vocational counseling, peer support, pre-discharge planning, and life skills training.
