community health workers
Community Health Workers are a community-based service that focuses on the continuum of care in regards to a person’s physical, mental, and emotional health. Community Health Workers work with low, moderate and high-risk populations within the community environment in order to cultivate knowledge and programs to support, produce, and maintain positive health outcomes. The program has some primary goals for health improvement including connecting citizens to primary care, providing resources to reduce diabetes and obesity for citizens served, enable citizens to self-manage chronic disease, locate healthcare services for individuals in the community and reduce the number of emergency room visits, and connect individuals to resources to address the social determinants of health.
